This form is needed for participation in any sport for the current school year. Section 1 - 5 - Parent/Guardian section - Section 6 - Doctor section.  

The form is filled out by parent/guardian if you play an additional sport for the current school year.  If you answer "yes" to any of the questions in the Supplemental Health History section, you must have a physician complete Section 9.

This form is filled out by a physician if you answered "yes" to any questions in the Supplemental Health History on  Section 8 and wish to participate in an additional sport during the current school year. 

This form is for any student that wants to participate in wrestling that needs to be filled out by a physician.

This form is filled and given to the head coach so that in an event that an injury should happen and the parent/guardian is not immediately available, the coach has accurate information. 

This information must be completed and returned to the Athletic office prior to participating in any sporting event.
